- please inculed a detailed description of the problem to the application,
- any documents that may be helpful in clarifying the case,
- if the application contains suggestions for improvements – indication of the current situation, existing difficulties and the postulated state and description of possible suggestions for improvements,
- information whether the case was previously reported to us and the addressee and the manner of settling the matter.
